  • CPT 96374 is often billed in conjunction with an appropriate E/M code, if a separately identifiable evaluation and management service is performed and documented. In that case, use modifier -25 with the E/M code.
  • Be sure to report the specific drug(s) administered using the appropriate HCPCS Level II “J-codes.”
  • This code is commonly used for medications such as antiemetics, corticosteroids, analgesics, antihistamines, or diagnostic agents delivered via IV.
